Registration:

Online registration for the January 2006 meeting is CLOSED. If you haven’t registered, you can register onsite on the first day of the meeting.
All participants must register for the meeting by clicking on the “Login” button under FDP Directory on the FDP website http://www.thefdp.org/ or if you are new to the FDP, you will need to establish a directory entry first by linking to Directory Entry for New Participants. After you successfully login, select the link for the Electronic Registration form for the January 2006 meeting. Once you submit your meeting registration an e-mail confirmation will be sent to you. Please print this confirmation as your meeting registration receipt. Note: If you have forgotten your password, you can select the link “Lost your password?.
Cancellations – Written notification is required for cancellations. Please e-mail Edvin Hernandez at ehernandez@nas.edu or fax your notification to 202.334.2725. Cancellations received after the deadline, December 16, 2005, will be charged an administrative fee of $25.
No Shows – Following the meeting, all no-shows will receive an invoice for $35.

Registration Fees:
|
|
Employees of federal agencies, faculty and staff from FDP member institutions, including Emerging Research Institutions, DO NOT have to pay a registration fee for meetings held at the National Academies’ in Washington, DC. This applies to anyone from a member institution who wishes to attend, not just your designated representatives.
Affiliate Fee – Registration fee for affiliate members is $75.
Non-Member/Vendor Fee – Registration fee for non-members/vendors is $100.
On-site registration – On-site registration fee for non-members/vendors is $125.
Checks should be made payable to FDP-MIT and should be mailed in advance to Edvin Hernandez at the address below. The Taxpayer ID number for MIT is 04-2103594 (if you require the signed W9 Form please send an email to fdp@nas.edu). Checks and cash will be accepted during on-site registration provided the participant has pre-registered.
We are unable to process credit card transactions.

All participants are responsible for making their own hotel and travel accommodations. Blocks of rooms have been reserved at the hotels listed below. All meeting participants must identify themselves as part of the Federal Demonstration Partnership group to receive the negotiated rate. The deadline for hotel reservations is December, 9, 2005. Rooms will be released after this date so please make your reservations early! Last minute reservations may be found at http://www.hoteldiscount.com.
